소아 연하장애 환자의 평가 및 치료

서론 : 소아의 연하장애는 성인의 연하장애와 구조적 특성이나 발달 요소 등에서 서로 다른 부분이 있다. 치료사는 소아의 발달에 따른 구조적 특성이나 발달요소를 정확하게 이해하고 소아의 발달 수준별 접근을 사용할 수 있어야 한다. 본 연구에서는 소아의 연하장애의 특성을 이해하고 소아의 연하장애 유형 및 평가방법, 그에 따른 치료법을 정리해 보고자 한다.

본론 : 삼킴의 발달은 소아가 경험하는 눈-손 협응의 발달, 정상 자세와 근 긴장도의 발달, 시기에 적절한 심리사회적 요소들에 의해 영향을 받는다. 해부학적으로 볼 때 성인과는 기능적인 차이점들이 있으며, 빨기, 삼키기, 호흡하기와 관련된 구조와 기능은 매우 밀접하게 관련되어 있을 뿐 아니라 서로의 기능이 중첩되어 이중적인 역할을 수행함으로 산소와 음식을 공급하게 된다. 소아에게 연하장애가 의심되어 평가를 의뢰하는 경우 관찰과 부모와의 인터뷰, 환경에 대한 반응과 상호작용, 각성 상태, 영상 평가와 후두 청진법을 통한 다면적인 평가를 수행하며, 연하장애의 치료는 다차원적 치료접근이 필요하다.

결론 : 소아의 연하장애는 소아의 발달 시기 및 임상적 징후에 맞는 적합한 평가를 사용하여 문제점을 정확히 파악해야 하며, 식사 활동에는 소아의 구강감각과 운동기술 능력 뿐 아니라 정신 사회적 능력 같은 수행요소 등을 포함한 다면적인 평가 및 치료접근이 필요하다.

 Evaluation and Intervention for Children With Dysphagia

Introduction :
Swallowing disorder of children is different with adult's one in structural characteristics or developmental factors etc. Therapist should understand the structural characteristics or developmental factors accurately in accordance with the child development and use approach by the child's developmental level. It was described the nature, type and how to evaluate of swallowing disorder in children. The treatment by result was also suggested in this study.

Main Subject :
The development of swallowing is affected by the development of eye-hand coordination, normal posture and muscle tone that child experienced and psychosocial factors. The child has the functional differences anatomically. Structures and functions the related suck, swallow and breathe are related very closely as well as provides oxygen and food through double role. In referral for evaluation, the most case are that whether food intake by mouth is safe and efficient method or whether is intaken appropriate food nutritionally. The evaluation is implemented through observation, interview with parents, response and interaction with environment, arousal state, VFSS and FEES. The treatment of swallowing disorder requires multi-dimensional therapeutic approach.

Conclusions :
In swallowing disorder of child, the problems should be identify by using appropriate assessment in accordance with child's developmental level and clinical signs.   The food activities for children need to multiple evaluation and treatment approach including  oral sensory and motor skills as well as psycho-social skills.
